Leading global health organizations and aid agencies are increasingly turning to AI-generated images depicting extreme poverty, vulnerable children, and survivors of sexual violence for their social media campaigns, sparking what experts are calling a new era of “poverty porn 2.0.” This troubling trend represents a significant departure from years of sector-wide discussions about ethical imagery and dignified storytelling.

According to global health professionals monitoring this development, these synthetic images are flooding major stock photo platforms and being adopted by prominent NGOs. “All over the place, people are using it,” confirmed Noah Arnold of Fairpicture, a Swiss organization promoting ethical imagery in global development. “Some are actively using AI imagery, and others, we know that they’re experimenting at least.”

Reinforcing Harmful Stereotypes

Arsenii Alenichev, a researcher at the Institute of Tropical Medicine in Antwerp who studies global health imagery production, has collected more than 100 AI-generated images of extreme poverty used in social media campaigns. “The images replicate the visual grammar of poverty – children with empty plates, cracked earth, stereotypical visuals,” he explained.

The images Alenichev shared reveal exaggerated, stereotype-perpetuating scenes: children huddled in muddy water; an African girl in a wedding dress with a tear staining her cheek. In a recent Lancet Global Health commentary, he argues these constitute “poverty porn 2.0,” building on long-standing concerns about ethical representation in humanitarian communications.

Drivers of the Trend: Cost and Consent

The proliferation of these images appears driven primarily by financial pressures and concerns about obtaining proper consent. “It is quite clear that various organizations are starting to consider synthetic images instead of real photography, because it’s cheap and you don’t need to bother with consent and everything,” said Alenichev.

Arnold noted that US funding cuts to NGO budgets have exacerbated the situation, forcing organizations to seek cheaper alternatives to professional photography. This shift comes amid broader industry developments in content creation that prioritize cost-efficiency over ethical considerations.

Stock Platform Proliferation

AI-generated poverty imagery now appears in significant numbers on popular stock photo sites including Adobe Stock Photos and Freepik. Search queries for “poverty” return dozens of synthetic images with captions like “Photorealistic kid in refugee camp” and “Asian children swim in a river full of waste.”

Adobe sells licenses to some of these problematic images for approximately £60 each. “They are so racialized,” Alenichev criticized. “They should never even let those be published because it’s like the worst stereotypes about Africa, or India, or you name it.”

Platform Responsibility Debate

Joaquín Abela, CEO of Freepik, shifted responsibility to media consumers rather than platforms. He explained that AI stock photos are generated by the platform’s global user community, who receive licensing fees when customers purchase their images.

While Freepik has attempted to address biases in other parts of its photo library by “injecting diversity” and ensuring gender balance in professional images, Abela acknowledged limitations: “It’s like trying to dry the ocean. We make an effort, but in reality, if customers worldwide want images a certain way, there is absolutely nothing that anyone can do.” This stance highlights ongoing challenges in market trends regarding content moderation.

Major Organizations Using AI Imagery

Several prominent organizations have already incorporated AI-generated imagery into their communications. In 2023, the Dutch arm of UK charity Plan International released a video campaign against child marriage featuring AI-generated images of a girl with a black eye, an older man, and a pregnant teenager.

More controversially, the UN posted a YouTube video with AI-generated “re-enactments” of sexual violence in conflict, including synthetic testimony from a Burundian woman describing being raped during the country’s civil war. The video was removed after media inquiries, with a UN Peacekeeping spokesperson stating it showed “improper use of AI” and posed risks regarding “information integrity.”

Ethical Backsliding

This trend represents a significant regression after years of progress toward more ethical representation. Kate Kardol, an NGO communications consultant, expressed concern: “It saddens me that the fight for more ethical representation of people experiencing poverty now extends to the unreal.”

The situation reflects broader challenges in related innovations where technological capabilities outpace ethical frameworks. Arnold noted the irony: “Supposedly, it’s easier to take ready-made AI visuals that come without consent, because it’s not real people.”

Amplification Risks

Generative AI tools have consistently been found to replicate and sometimes exaggerate societal biases. Alenichev warned that the proliferation of biased images in global health communications could create a dangerous feedback loop: these images may filter into the wider internet and be used to train future AI models, further amplifying prejudice.

This concern is particularly relevant given recent technology developments in AI training data collection, where problematic content can become embedded in systems.

Organizational Responses

Some organizations are beginning to respond to criticism. A spokesperson for Plan International stated the NGO had, as of this year, “adopted guidance advising against using AI to depict individual children,” and explained that their 2023 campaign used AI-generated imagery to safeguard “the privacy and dignity of real girls.”

However, without industry-wide standards and greater platform accountability, experts worry the problem will continue to grow, potentially undermining years of progress in ethical humanitarian storytelling and representation.
